If I am out-of-network with your insurance, I will provide you with a monthly receipt for all services rendered that may be submitted for insurance reimbursement for clients who have PPO plans with out-of-network benefits, and will assist clients with submitting insurance when necessary.

ALL co-payments for in-network services are due at the time that services are rendered.
